Before your first visit
A first appointment covers your history, an examination, and any imaging you already have. Bringing the following helps us use the time well.
- Photo ID
- Insurance card
- Referral letter, if your plan requires one
- Imaging and reports from other providers
- A current list of your medications
Please arrive early enough to complete registration and a short health questionnaire.
Changing or cancelling an appointment
Call the office during business hours with as much notice as you can, so the appointment time can be offered to another patient who is waiting.
