How do I schedule an appointment?

Call our office at 904-204-5000 so we can schedule a time and date that’s convenient for you. You’ll need to complete a medical and health history and other forms before your appointment. Please arrive 30 minutes before your scheduled appointment to complete the paperwork in our office.

Do I need a referral to make an appointment?

Many health insurance plans require that you obtain a referral from your primary care provider before seeing a specialist like an orthopaedic surgeon.

Will my insurance pay for any medical services?

Health insurance plans cover most orthopaedic services. However, every insurance plan is different, and your financial responsibility depends on your specific coverage. To get information on your plan’s overage, please check with your insurance company or contact our office with your insurance information. Rest assured, we always obtain pre-authorization for any surgery and hospital stay.

What should I bring with me when I come for my initial appointment?

  • Your Insurance card
  • Co-payment if required by insurance (cash, all major credit cards, and personal checks accepted)
  • Your referral paperwork if required by your insurance and JOI does not already have it on file
  • Medical records from your other health care providers, if you have them
  • Films or a CD of CT scans or MRI and actual X-ray films (not just paper reports) or test results. Please ask your referring physician to help you obtain actual images. It is up to you to hand carry those images to the appointment.
